CONTINUING EDUCATION

The ACMA 2024 Leadership & Physician Advisor Conference will offer continuing education (CE/CME credits) and learning for physicians, social workers and nurses.

Conference Objectives:

  1. Explore innovative care delivery approaches and alternative models for transitional care management and transformation. 

  2. Apply case examples and best practices to advocate for vulnerable populations and address the needs of patients with complex care.

  3. Identify strategies, infrastructure, and analytics to advance practice and promote workforce resiliency to meet healthcare needs.

  4. Manage complex relationships and develop collaborative partnerships between healthcare stakeholders, patients, families, and providers.

  5. Apply clinical judgment and documentation review to validate medical necessity, appropriateness of level of care and resource utilization.

EARN UP TO THE FOLLOWING CE HOURS

We anticipate that you may earn up to the following continuing education (CE/CME hours) for the Pre-Conference and Main Conference.

  • 14.0 | Nursing and Social Work Contact Hours (50 or 60-minute hour)
  • 14.0 | ACM & CMAC Recertification Credits
  • 20.0 | CMPC Recertification Credits
  • 20.0 | AMA PRA Category 1 Credits™

CCM - Through a collaborative agreement with the Commission for Case Manager Certification (CCMC) this activity and the corresponding ACM/CMAC credits are acceptable for CCM renewal.

METHOD OF PARTICIPATION AND REQUEST FOR CREDIT 
Attendee Completion Requirements

  • Must attend full session.
  • Must complete/submit an electronic evaluation.
  • No partial credit will be given.

Request for Credit (All Disciplines)
You will receive an email from ACMA 1-3 days post-conference notifying you that the electronic evaluation is available. Detailed login instructions will be listed in the email. Once you log in and complete the online evaluation, you will have immediate access to download and/or print your CE Certificate.

As required by the approval agencies listed above, ACMA maintains records and CE certificates for all educational activities for a minimum of seven years after the educational activity concludes.
